Process for Requesting, Dispersing, and Tracking Gun Locks for SERTAC
· Members of SERTAC are eligible to request gun locks.
· Requests must be made through the official SERTAC JotForm request form
· Click the link – https://form.jotform.com/251274081302143
· Fill out the request form, providing necessary details such as purpose, quantity, and contact information.
· Submit the form for approval.
· Requests will be reviewed by the SERTAC within 5 business days.
· Approval notifications will be sent via email.
· Once a request is approved, the requester will receive an email confirmation.
· Requests are asked to work with SERTAC to coordinate either delivery or pick up of the gun locks
· Members are required to report on the usage of gun locks quarterly.
· Reports should be done with this JotForm link. https://form.jotform.com/251274433382051
· Failure to report may result in restrictions on future requests.
· In case of lost or damaged gun locks, members should immediately report the issue to Tom at sertacwi@gmail.com
· Replacement requests will follow the same approval process as new requests.
